TRAIN Seattle Waiver and Release of Liability In consideration for being allowed to use the facilities and services provided by TRAIN Seattle, a gym operated by Re:Turn Fitness LLC, I, the undersigned, acknowledge, appreciate, and agree to the following terms and conditions: 1. Assumption of Risk I am fully aware that engaging in physical activity, including but not limited to exercise, weightlifting, cardiovascular training, and the use of exercise equipment, involves inherent risks of physical injury or death. I understand that these risks may arise due to my own actions, the actions or inactions of others, the condition of the facilities, equipment, or premises, and other factors. 2. Health and Medical Condition I represent and warrant that I am physically able to participate in physical activities and have no medical conditions that would prevent my participation. If I have any health concerns or medical conditions, I have consulted a medical professional before participating in any physical activities at the Gym. 3. Release and Waiver I hereby voluntarily release and forever discharge TRAIN Seattle, by Re:Turn Fitness LLC, its owners, employees, contractors, trainers, and representatives (collectively referred to as "the Gym") from any and all claims, liabilities, demands, actions, causes of action, costs, and expenses, whether at law or in equity, arising out of or related to any injury, loss, or damage that may be sustained by me or caused to any property belonging to me, arising from my participation in activities at the Gym. 4. Indemnification I agree to indemnify and hold harmless the Gym from any and all claims or liabilities, including those of third parties, arising out of or related to my participation in Gym activities. 5. Agreement to Gym Policies I acknowledge that I have read, understood, and agree to all of TRAIN Seattle's policies. I understand that I can request a written copy of these policies by emailing info@trainsea.com. 6. Acknowledgment and Acceptance I have read this liability waiver and fully understand its terms. I understand that I am giving up substantial rights, including the right to sue. I acknowledge that I am signing this waiver freely and voluntarily. I also agree to the use of electronic signatures for this form. Filling out this form in its entirety with my name below and selecting "AGREE" constitutes my electronic signature.
